Due to the unprecedented public health crisis caused by COVID-19, Governor Kate Brown and her administration have ordered that all Oregon K-12 schools be closed to students March 16 through April 28. During this time all school activities, including athletic events and practices, are canceled. This is a preventative measure to help slow the spread of COVID-19 (also known as “novel coronavirus”) and its impacts on our citizens and health care systems. 

THERE WILL BE NO PRACTICES FOR SPORTS MARCH 13-APRIL 28. 

 

 

Azalea Athletic Paperwork

Brookings Harbor School District uses an Electronic Athletic Form. The account you set up now, will be used all through High School! 

Physicals require a paper copy turned into the school. 
Physicians MUST use the district approved physical form.  (See link below) 

DO NOT WAIT UNTIL THE LAST MINUTE.  The office closes at 3:45.  Please allow enough time for the office to process your registration & notify the coach of eligibility.

What you'll need for Athletic registration: 

  • Person to contact in case of emergency (Other than parents)
  • Insurance Information
  • SIGN UP FOR ALL SPORTS YOUR STUDENT MAY BE INTERESTED IN PARTICIPATING. 
 

****If your student does not have insurance, you can purchase some at http://www.k12StudentInsurance.com    You will need to have your student ID number, and a credit card.  Print off the confirmation and bring it to the office.
 

Your student must be signed up through FamilyID (above) and have an updated physical BEFORE they can participate. The student must be cleared by their doctor. Physicals are good for two years. Click the link below to print out a physical form.

Athletic Fee Payments

$65.00 Athletics fees must be paid before the first game or event.  ALL PAYMENTS MUST BE PAID AT THE FRONT OFFICE. We accept cash, check, or card. 

If you cannot make the payment in full,  you can print off the payment plan sheet below, fill it out, and return it to the office. 

ALL SCHEDULES ARE SUBJECT TO CHANGE.

Cross Country
This Fall sport starts the summer before school starts. Practice at 4 on the track:

Practice Monday-Friday 3:30-5:30
Boys Basketball
2020 tryout's for 7th and 8th grade boys are January 6-7 from 3:30-6pm in the AMS Gym. Any 6th grade students wanting to play basketball are encouraged to join the Brookings community league by signing up at  http://brookingsyouthactivities.com/ now through 12/20. Please contact Ashley at AMS with any questions 541-469-7427 opt 3.
 
Boys practice M-F 3:30-6:00pm in the AMS Gym.
Cheer/Dance
2020-2021 season begins in September 2020
 
Wrestling
Begins October 21, 2019
Practice Monday-Friday 3:30-4:30 in the small gym. Wednesday's practice begins at 2:30.
 
Starting March 23, all practices will be held in the cafeteria. 
 
March 7th there is a wrestling tournament at Crater High School. The bus will depart at 1:00 pm, with weigh-ins beginning at 4:00 pm. The top three in their weight class will advance to State finals on March 14th. If you have any questions please contact Mrs. Ashley at 541-469-7427, option 3.
Track and Field
March 12th will be the beginning of track season! Make you’re signed up on FamilyID and have your physicals in before that date. If you have any questions, please see Ms. Ashley in Student Services.
 
Practices will be Monday-Friday 3:30-5:15.
 
No practice Spring Break 3/23-2/27.

Changes in Transportation from Athletic Events

NEEDED:
A letter from the parent or guardian of the student stating :

  • Student's name
  • Date of the event
  • Who is taking the student from the event
  • Phone number
  • Parent or Guardian printed name & Signature
 

NEEDED:
A letter from the person taking the student stating:  

  • Your name
  • that you are taking responsibility for Student's name
  • Date of the event
  • Printed name & signature
 

Staff needs to have BOTH letters approved and signed by the vice principal, make a copy, and give it to the coach BEFORE departure. 

If any of these steps are missing the student will need to take the school bus home from the event.  NO EXCEPTIONS.
